Quilt Entry Registration
1. Print and complete the Quilt Entry Form. A separate Quilt Entry Form is required for each submission.
2. Enclose your Quilt Entry Form(s) in an envelope, along with your check or money order made payable to the Glass City Quilt Commission, and mail to:

NOTE: If you include an email address on your form(s), you will receive an email confirmation of your registration(s). If you would like to receive written notification of the receipt of your quilt show entry and/or workshop registration and payment, please enclose a self addressed stamped envelope with these forms and payment.
All entry forms and fees must be submitted and received by July 6, 2017.
